How to contribute to the survey

The questionnaire software for this survey should be downloaded for data entry on your computer. The program contains all the necessary electronic forms for data entry into each questionnaire category. Once the data has been entered the program will produce a specially formatted data file for you to send to us on diskette or as an email attachment. On receipt of this file we can import your data directly into our database.

Any contributors wishing to provide information for one or more of the dams listed may do so by downloading the installation files.

Preparatory and Supporting Documents:  (You may wish to print these for reference)

• Firstly, please review the list of the dams included in the survey to verify that the dam(s) about which you have information are included in the survey.
• Secondly, view the guide to completing the questionnaire which explains how to complete the questionnaire and use the software.
  This can also be downloaded as a Word 97 document (22kb)
• A version of the questionnaire is available as a printable web page [101kb]. This may be useful for making notes on hard copy before entering data into the questionnaire software.
  You can also download the questionnaire as an Excel 97 workbook - [593kb]
• A short Supplementary Questionnaire consisting of 13 additional questions has been developed and we would be grateful if you could complete this and fax/email it to the commission (these extra questions are not included in the questionnaire software).

  The Supplementary questions are available here in three formats, all suitable for output on a desktop printer:

  • A web / HTML version - (18k)
  • An Adobe Acrobat PDF - (15k)
  • An MS Word97 document - (42k)

Please read the Installation Instructions carefully before you install this program. Please view or download the Questionnaire Guide Document - this takes you through the program step-by-step and informs you how to distribute the completed questionnaire back to the World Commissions on Dams Secretariat.

  Click on the link above to save the installation file to a directory on your computer's hard disk, when prompted to save the file please name it to wcdsurv.exe (not *.zip) then consult the installation instructions to set up the software on your system.

Please note that this software requires Microsoft Windows 95 / 98 / NT. If you wish to contribute information but are unable to gain access to a PC running Windows please print out the web version of the questionnaire and enter the data clearly, then fax this hard copy to the WCD at - +27 21 426 0036
